ЭФФЕКТИВНОСТЬ РЕСПИРАТОРНОЙ ПОДДЕРЖКИ И N-АЦЕТИЛЦИСТЕИНА У ПАЦИЕНТОВ С ОСТРОЙ ДЫХАТЕЛЬНОЙ НЕДОСТАТОЧНОСТЬЮ НА ФОНЕ ПОЛИТРАВМЫ

Цель работы — сравнительное изучение эффективности применения амброксола и N-ацетилцистеина (АЦЦ) у пострадавших с ушибом легких и острым респираторным дистресс-синдромом (ОРДС). Обследовано 38 пациентов. Интенсивная терапия соответствовала принципам лечения при ОРДС. Пациенты контрольной группы получали для улучшения респираторной функции легких амброксол. Пациенты основной группы получали АЦЦ. Изучалось напряжение газов крови, содержание в крови карбоксилированного гемоглобина (HbCO %), центральная гемодинамика, содержание жидкости в грудной клетке, показатели статического дыхательного комплаенса, инспираторного сопротивления дыхательных путей в течение 5 суток интенсивной терапии. Терапия АЦЦ достоверно ускоряла нормализацию напряжения газов крови, уменьшение содержания HbCO %, улучшение производительности сердца, уменьшение накопления жидкости в грудной клетке. Респираторная поддержка с созданием в дыхательных путях постоянного положительного давления, способствовала сохранению величины дыхательного комплаєнса, даже в условиях тяжелой гипоксемии и накопления жидкости в грудной клетке.
